All requests for consideration of grants should be submitted to:

Please add this sheet to your application and place a check mark in each box to indicate the information has been included. Grants applications MUST INCLUDE the following:

Type of award applying for:
___ Project Grant
___ General Operating Grant
___ Capital Grant
___ Ph.D./Veterinary Fellowship

REQUIRED Application Information:
___ Name and address of organization.
___ Name of current director and telephone number.
___ Name of contact person (if not the director) and telephone number.
___ Brief history of the organization
___ Current list of board members.
___ Reason for the request for funds
___ Amount requested.
___ Financial breakdown or a statement of expected use of funds.
___ Last audited financial report of the organization.
___ Proof that the organization has received 501(c)3 status, and is not a "private foundation."
___ Summary statement MUST be included in proposal.
___ SIX COPIES OF THE ENTIRE PROPOSAL

Application Deadlines:

May 1st for Project, Operating, and Capital Grants
December 1st for Ph.D./Veterinary Fellowships (applications should be received by December 1st and the grant cycle will start after June 1st of the following year)

In order to be considered for funding, ALL REQUIRED INFORMATION must be in the Gaithersburg Office BEFORE the established deadline.

DO NOT PLACE YOUR APPLICATION IN PLASTIC OR CARDBOARD
FOLDERS OR INCLUDE A LARGE AMOUNT OF EXTRANEOUS
MATERIAL SUCH AS NEWSLETTERS OR NEWSPAPER ARTICLES.

YOUR APPLICATION WILL NOT BE CONSIDERED!
